What do I need to complete an application

To complete the application the following must accompany this form:

• An original or duly certified Police check, less than three (3) months old.
• complete all of the relevant sections of the application form

Late renewal applications

A licence may be renewed for applications lodged within 28 days of expiry of the licence so long as any required fee and penalty for the late application has been paid.
