This form is used to apply for voluntary deregistration of a co-operative under the Co-operatives Act 2009 (the Act) provided it meets certain criteria. Once deregistered, the co-operative ceases to exist as a legal entity and can no longer do anything in its own right.
Before applying co-operatives need to ensure that they meet the following requirements:
Applications that do not meet these requirements (including lodgement of due annual returns) and are not signed by the co-operatives director or secretary will be rejected.

Download the form and after completing post it to: Associations and Charities Pay by credit card or BPAY: Once we receive your form you will be issued with a Payment Number (PN) so you can make payment using our secure online payment portal. Pay by cheque or money order: Attach a cheque or money order made out to "Department of Energy, Mines, Industry Regulation and Safety" to the completed form. |
