On-the-job Training Record Book – Class G Gasfitting Permit

This application form is suitable for a Western Australian indentured apprentice or non-apprentice (trainee) who has completed either a plumbing apprenticeship or a Certificate III in gasfitting plus on-the-job training and Toolbox session. If either the on-the-job training or Toolbox session have not been completed, please use the Application for a Class G gasfitting permit – tradesperson restricted to work under supervision.
