Starting a Co-operative

Form 01 Application for approval of proposed rules of a co-operative

This form is used to obtain approval of the rules and disclosure statement (if applicable) prior to the formation meeting.

Form 03 Application for registration as a co-operative

This form is used to register a co-operative following the formation meeting.

You must also complete and attach the Co-operative statement of directors' details to the Form 3.

Changing the rule forms

Form 30 Co-operative Application for approval of proposed alteration to the rules

This form is used to obtain prior approval to amending rules concerning active membership or conversion of a non-distributing co-operative to a distributing co-operative.

Form 32 Co-operatives application for registration of an alteration of rules

This form is used to register an amendment to the rules of the co-operative. The application must be made within 28 days of the amendment being passed.

You must also complete and attach the Statutory declaration in support of an alteration of the rules to the Form 32.

Form 10 Co-operatives annual return to the registrar

This form is used by Co-operatives to lodge their annual return to the Registrar.

Form 12 Co-operatives application for extension or abridgement of time

This form is used to obtain approval for an extension or shortening of specified time frames in which to undertake or complete required activities.

Form 13 Co-operatives notice of appointment or cessation of directors and officers

This form is used to notify the appointment, cessation or change of details of a co-operative’s secretary, chief executive officer or director.

Form 14 Co-operatives change of registered office

This form is used to notify the change of the co-operative’s registered office.

Form 24 Notification of resignation, removal or cessation of an auditor

This form is used to notify the resignation, removal or cessation of an auditor.

Form 35 Application for registration of a special resolution (other than for altering the rules)

This form is used to register special resolutions passed by the co-operative.

You must also complete and attach the Statutory Declaration in support of a special resolution not altering rules to the Form 35.

From 1 November 2023, we will no longer accept lodgement fees by credit card for co-operative forms submitted by post.

If you intend to pay by credit card, once we receive your form you will be issued with a Payment Number (PN) so that you can make payment using our secure online payment portal.

Alternatively you can take your completed form and pay by credit card in person at our Cannington office or any of our regional offices.
